How to use the Mounjaro KwikPen

The KwikPen holds four fixed weekly doses. Each week you fit a new needle, prime the pen, dial the full dose and inject under the skin. This guide follows the official Spanish leaflet, step by step.

  • 4fixed doses in each pen
  • 1×injection a week
  • Primebefore every injection
  • 30days at most after first use

These instructions do not replace a conversation with your doctor, nurse or pharmacist about your treatment.

Mounjaro KwikPen

Know your pen

One pen lasts four weeks. The needles are not in the box, so have everything ready before injection day.

Mounjaro KwikPen with its cap on, a pen needle, gauze and a small sharps container

The pen

  1. Pen capProtects the pen. Under it is the red inner seal you wipe before fitting the needle.
  2. LabelCheck the name and strength match your prescription.
  3. Dose windowShows the start position and the full-dose symbol.
  4. Dose knobTurn it to set the start position or the dose, push it to prime and inject.

What you need each week

  1. A new pen needleOne compatible with KwikPen, new every time. Not in the box: ask your pharmacist which one to use.
  2. A swab, gauze or cotton ballTo wipe the red inner seal, and to press lightly if there is a little blood.
  3. A sharps containerOr a hard plastic container with a secure lid, for used needles.

The weekly injection, step by step

Seventeen short steps in four stages. Read the leaflet's instructions each time you start a new pen, in case anything has changed.

Prepare

steps 1 to 6
  1. Wash your hands with soap and water.
  2. Pull off the pen cap and check the pen. Do not use it if the name or strength does not match your prescription, if it is out of date or damaged, or if the medicine has frozen, has particles, is cloudy or discoloured. It should be colourless to slightly yellow.
  3. Wipe the red inner seal with a swab.
  4. Take a new pen needle and pull off its paper tab.
  5. Push the capped needle straight onto the pen and turn it until it is tight.
  6. Pull off the outer needle shield and keep it. Pull off the inner shield and throw it away.

Prime the pen

steps 7 to 9, every week
  1. Turn the dose knob slowly until you hear 2 clicks and the long line shows in the dose window. This is the start position. If it is not aligned, turn the knob either way until it lines up with the dose indicator.
  2. Hold the pen with the needle pointing up and tap the cartridge holder gently to move air bubbles to the top.
  3. Push the dose knob in until it stops, then hold it and count slowly to 5. A little medicine should come out of the needle tip. Do not inject this into your body.
No medicine at the needle tip?
  1. Repeat steps 7 to 9, no more than 2 more times.
  2. Still nothing: change the needle and repeat steps 7 to 9 once more.
  3. Still nothing: contact Lilly, using the details in the leaflet, or ask your pharmacist.
The KwikPen held straight against the skin of the abdomen, well away from the navel, thumb on the dose knob

Inject

steps 10 to 13
  1. Choose the injection site: the thigh or the abdomen, at least 5 cm from the navel. Someone else can inject into the back of your upper arm.
  2. Turn the dose knob until it stops and the full-dose symbol shows in the dose window.
  3. Insert the needle into your skin. Push the dose knob in until it stops, then hold it and count slowly to 5. Check the symbol in the dose window before you take the needle out.
  4. Take the needle out. A drop of medicine at the tip is normal and does not affect your dose.
Not sure you got the full dose?If the window does not show the symbol, put the needle back into your skin and finish the injection. Do not dial the dose again. If you still think you did not get the full dose, do not start again or repeat the injection: speak to your doctor or pharmacist.

After the injection

steps 14 to 17
  1. If there is blood, press lightly on the site with gauze or cotton. Do not rub.
  2. Carefully put the outer needle shield back on.
  3. Unscrew the capped needle and put it in the sharps container. Never store the pen with the needle on: it can leak, block the needle and let air in.
  4. Put the pen cap back on. Do not store the pen without its cap.

Where to inject

Mounjaro goes under the skin, never into a vein or muscle. You can use the same area each week, but choose a different spot within it.

Front and back of a human body with the injection sites marked: abdomen around the navel, thighs, and the back of the upper arms
FrontBack
  • Abdomen

At least 5 cm away from the navel. You can inject here yourself.

  • Thigh

The upper leg. You can inject here yourself.

  • Back of the upper arm

Only with help: another person should give the injection here.

If you also inject insulin, use a different site for it.

Storage and when to throw the pen away

Keep pens and needles out of sight and reach of children. Never freeze the pen: if it has frozen, throw it away.

Before first use

2–8 °C

In the fridge. Unused pens can be used until the expiry date on the label.

After the first injection

below 30 °C

The pen in use can stay at room temperature. Always with the cap on and without a needle.

Throw it away

4 doses or 30 days

After the 4th weekly dose, or 30 days after first use, even if medicine is left. A little leftover is normal and keeps the pen working: never try to inject it.

If something goes wrong

Answers from the official leaflet. For anything else, ask your doctor, nurse or pharmacist.

The pen cap will not come off

Twist it gently back and forth, then pull it off.

I cannot turn the knob to the full dose

Throw the pen away, including the medicine left in it. There may not be enough for a full dose. Do not try to inject the leftover medicine.

The dose knob is hard to push

Pushing more slowly makes it easier. The needle may be blocked: fit a new needle and prime the pen. If dust, food or liquid has got into the pen, throw it away and use a new one.

There is a drop at the needle tip after the injection

That is normal and does not affect your dose.

I missed my weekly dose

If 4 days or fewer have passed, inject as soon as you remember and then go back to your usual day. If more than 4 days have passed, skip the missed dose and inject on your next usual day.

Can I change my injection day?

Yes, as long as at least 3 days have passed since your last injection. Then keep injecting once a week on the new day. You can inject at any time of day, with or without food.

Can I share my pen?

No, not even with a new needle. Sharing can pass on a serious infection.
