Medical document after assessment
Explain how illness is affecting your work.

A doctor assesses your symptoms and work situation in English. If clinically appropriate, they may issue a private medical note to explain the assessment.
Medical decisions are made by the clinician after assessment. Not an emergency service.
The document follows the medical decision.
The appointment starts with your health, not a pre-written note. The doctor asks when symptoms began, how they have changed, what your work involves and whether you need treatment, rest or further assessment.
If the findings support it, a private note may describe the assessment. This is different from an official Spanish social-security sick-leave certificate (parte de baja), which follows its own public or mutual-insurance process. Check what your employer or requesting organisation will accept.
Explain the illness and when it began.
Tell the doctor how duties are affected.
A note depends on the clinical assessment.
How the sick note assessment works
Clear expectations before you choose a time.
Before the appointment
Have the dates your symptoms began, your work duties, relevant medication and any medical information ready. Ask your employer what type of document it requires.
During the assessment
The doctor asks about symptoms, their effect on work and any warning signs. They may recommend treatment, further investigation or urgent care where needed.
After the assessment
If a private note is clinically justified, the team explains the document and delivery. If you need an official Spanish sick-leave form, the doctor can explain that a separate authorised route applies.
Good to know
Private note or official baja?
A private medical note is not automatically the official Spanish parte de baja for statutory sick leave. Confirm which document your employer needs before booking.

Practical answers about this service.
Will I definitely receive a sick note?
No. A doctor issues a private note only when it is clinically appropriate after assessing you.
Is this an official Spanish baja laboral?
No. Official sick-leave certificates follow the Social Security process through the authorised public health or mutual-insurance route.
What should I prepare?
Know when symptoms started, how they affect work and what type of evidence your employer has requested.
